New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St.

New Horizons is a nonprofit agency seeking a caring and compassionate Care Coordinator to assist agency/organizational staff and/or clients by coordinating treatment and support services.
As a Care Coordinator, you will provide care coordination to individuals seeking outpatient services at New Horizons of the Treasure Coast, Post-acute care treatment at a local Baker Act receiving facility (Cleveland Clinic Behavioral Health Center).

Acts as a liaison between local Baker Act receiving facility (Cleveland Clinic Behavioral Health Center and New Horizons of the Treasure Coast), Outpatient services department Provides time-limited (two weeks) care coordination for individuals discharging from Cleveland Clinic Behavioral Health Center.
Ensures individuals are engaged in their current setting (e.
g.
, crisis stabilization unit (CSU), SMHTF, homeless shelter, detoxification unit, addiction receiving facility, etc.
) to establish a warm hand-off.
Models, coaches, and supports shared decision-making in care planning and service determination with the individual and family members (where applicable) and emphasizes self-management, recovery and wellness, including transition to community-based services and/or supports.
Helps develop procedures and protocols for information sharing between New Horizons of the Treasure Coast and Cleveland Clinic Indian River Behavioral Health Center.
Coordinates with NHTC SOAR Specialist to assess individuals for eligibility of Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Veteran's Administration benefits, housing benefits, and public benefits, and assist them in obtaining eligible benefits, including the use of SOAR when assessing for SSI and SSDI.

Qualifications:
Bachelor's Degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field two years full time or equivalent experience working with adults and children at risk Valid and current Florida Driver's License required with no more than 6 points in the past 5 years.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
