Currently seeking a full time Patient Care Manager to work with our Home Health Team in the Florida Ridge area.
The ideal candidate must hold current licensure as an RN in the State of Florida.
The responsibilities of the Clinical Manager Home Health Daily crisis intervention, problem solving and counseling.
Responsible for determining staffing needs and coordinating the interviewing and hiring of new nurses with Human Resources and the Program Director.
Responsible for understanding State, Federal, JCAHO regulation and agency policies, and being a resource to staff regarding regulations and policies.
Audits charts for quality assurance.
Establishes QA monitors for nursing field staff communicates results to nurses.
Helps to establish plan of action if monitors indicate the necessity.
Develops nurses in their management skills i.

, prioritizing, time management, and problem solving.
Conducts yearly performance appraisals of nursing staff.
Plans, coordinates and conducts all nursing/team meetings and is responsible for ensuring that agency happenings/changes are communicated to nursing staff.
Conducts and documents case conferences with interdisciplinary team, and participates in QM meetings and Professional Advisory Committee.
Handles any calls to patients and physicians when needed for conflict resolution, or to promote a new program or already existing services.
Rotates through supervisor on-call.
In home supervision of RNs & LPNs.
Analyzes care regimens and evaluates care based upon current policies and standards of practice.
Requirement of the Clinical Manager Home Health Two years previous home health and supervisory experience required Must be currently licensed as an RN in the state of Florida Compensation of the Clinical Manager Home Health Salary based on experience with benefit package Job ID:
